Responsible for conducting retrospective and applicable concurrent medical record reviews. Abstracts defined quality measures, and serves as a resource for organizational quality and performance improvement for professional staff, nursing, clinical support services, and ancillary services. Reports to the Director of Quality and Advocacy Services.

The overall responsibilities of the Case Manager include facilitating the achievement of client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation. Based on the needs and values of the client, and in collaboration with all service providers, the case manager links patients with appropriate providers and resources throughout the continuum of health and human services and care settings, while ensuring that the care provided is safe, effective, client-centered, timely, efficient and equitable. This approach achieves optimum value and desirable outcomes for all – the patients, their support systems, the providers, and the payers.

Current licensure and registration as a Registered Professional Nurse in the state of Illinois. Minimum three years of progressively more responsible work experience in appropriate clinical areas in order to gain knowledge of regulatory agency requirements and Graham Hospital Association policies and procedures. Graham Health System is seeking candidates for the Vice President of Clinic Operations/Medical Staff Affairs. Responsibilities include the day-to-day leadership of Graham Medical Group (GMG), a 28 provider multi-specialty practice, located in 4 geographic locations. Graham Hospital has 35+ Active and 100 Affiliate Medical Staff members. Board Certification is a requirement of Medical Staff Membership. GMG has witnessed significant growth in the past 4 years and will be moving into a state-of-the-art 68,000 sq. foot clinic in September of 2010. In 2008, GMG implemented a fully functional electronic medical record and integrated practice management system.

Interested candidates must have significant clinic operations management/leadership experience in a Medical Group Practice or proven experience in physician relations. Candidates should also have experience with physician recruitment, hospital credentialing and privileging, finance, marketing and human resources experience, and strong strategic planning skills. Master’s Degree in Health Administration or Business Administration preferred.

The Exercise Specialist is bachelor's prepared in exercise science and is responsible, in conjunction with the department head, for developing and overseeing safe and effective exercise programs for all populations seen at the Graham Wellness Center. The Exercise Specialist educates program participants in means of safe and effective home and rehab exercise and activities and is well versed in all cardiopulmonary disease risk factors. The Exercise Specialist communicates participant exercise responses to the physician by program summary reports and telephone calls. The Exercise Specialist collects outcome data measurements and assists department head in program and IOP development.

Must have Bachelors degree in Exercise Science. Must have EKG interpretation course and skills to interpret telemetry monitor. Must have Advanced Cardiac Life Support Certification. ACSM Exercise Specialist Certification desirable. Must be capable of and have stamina to perform exercise leadership and assess the exercising patient.
