Graham Health System assures that each patient’s medical record is kept confidential and secure. Confidentiality is a major priority. A primary responsibility of our medical records staff is to fulfill requests for health information from patients, insurers, physicians, and other healthcare providers.
Accessing Medical Records
Your hospital medical records can be accessed by logging into the MyGraham Patient Portal. It is a website that allows you to access information about your hospital stay and test results. Patients may review instructions, test results, and your most recent discharge medication list. Patients may request access by emailing firstname.lastname@example.org. Graham Health System representatives are happy to assist patients in obtaining access to the MyGraham Patient Portal and can be reached at (309) 649-6805 Monday – Friday between 7:00a.m. and 3:30p.m.
Your physician office records can be accessed by logging into Patient Portal. This website allows patients to access information regarding Graham Medical Group clinic visits. Patients may view lab and imaging results along with scheduled appointments. Graham Health System representatives are available to assist patients with any questions regarding access and can be reached at (309) 649-6805 Monday – Friday between 7:00 a.m. and 3:30 p.m.
Requesting Medical Records
In order to protect your privacy, we require written authorization before medical records are released. To obtain copies of medical records, patients will need to complete an Authorization for Release of Information. The authorization must be completed, signed, and dated by the patient or the patient’s legal representative before copies will be provided. The authorization can be sent by

Please allow five to ten business days to process your request. Records will be mailed to the address placed on the Authorization for Release of Information form. Records may also be picked up if prior arrangements are made with the Health Information Management department. To protect health information, a valid picture identification card (passport, driver’s license, state ID, etc.) must be presented when picking up medical records in person.
The Health Information Management Department is open 8:00 a.m. – 4:30 p.m. Monday through Friday. Questions regarding medical records can be answered by calling:

Copies of birth or death certificates must be obtained through the Fulton County Clerk’s Office
