Health Information Management
The Graham Health System Health Information Management (HIM) employees are responsible to ensure each patient’s medical record is kept confidential and secure. In addition to maintaining medical records, our staff fulfill requests for health information from patients, insurers, physicians, and other healthcare providers.
Accessing Medical Records
Patients may access their hospital medical records by logging into the MyGraham Patient Portal. This website allows many of our patients to access information regarding hospital stays, test results, specific instructions our patients may have been given, and may even include the patient’s most recent discharge medication list. Graham Health System medical records representatives are happy to assist our patients to obtain access to the MyGraham Patient Portal and can be reached at (309) 649-6805 Monday – Friday between the hours of 8:00a.m. and 4:30p.m.
Your physician office records may also be accessed by logging into the Graham Medical Group Patient Portal. This website allows patients 18 or older to access information regarding Graham Medical Group scheduled appointments, past clinic visits, as well as lab and imaging test results your Graham Medical Group physician may have ordered. Our Graham Medical Group Scheduling and Registration Departments are available to assist patients with any questions regarding access and can be reached at (309) 647-0201 Monday – Friday between the hours of 8:30 a.m. and 4:30 p.m.
Requesting Medical Records
In order to protect your privacy, we require written authorization before medical records are released.
To obtain copies of medical records, patients will need to complete an Authorization for Release of Information. The authorization must be completed, signed, and dated by the patient or the patient’s legal representative before copies will be released. The authorization may be sent to Graham Health Information Management one of three ways:

Please allow five to ten business days to process your request. Records will be mailed to the address placed on the Release of Information Authorization form. Records may also be picked up in the Health Information Management Department if prior arrangements are made with our HIM staff. To protect health information, a valid picture identification card (passport, driver’s license, state ID, etc.) must be presented when picking up medical records in person.
The Health Information Management Department is open 8 a.m. – 4:30 p.m. Monday through Friday. Questions regarding medical records can be answered by calling:

Copies of birth or death certificates must be obtained through the Fulton County Clerk’s Office
(309) 547-3041 extension 306 in Lewistown or at the Fulton County Health Department (309) 647-1134 in Canton.
Charge for Medical Records
Graham Health System follows the Health Insurance Portability and Accountability Act and the State of Illinois fee schedule for costs associated with the copying of medical records. There is no charge to send patient information to another physician/health care facility to ensure the continuum of patient care.
