What is the Health & Wellness Connection?
The Health & Wellness Connection of Fulton County (HWCFC) is a care management facility that works closely with Graham Health System to provide qualified individuals with low or no cost health services. Individuals who need assistance with things like medications, diabetic supplies, vision and dental care and transportation to seek medical care may qualify for financial assistance. Individuals who do not qualify may seek assistance for referrals to other appropriate organizations.
Our Goals:
In Fulton County, far too many of our neighbors don’t have these essentials. The rising cost of living means many families are making impossible choices—skipping dental visits, delaying eye exams, or cutting back on medications - just to keep food on the table. And for those who are uninsured or underinsured, the challenges are even greater. That’s where the Health & Wellness Connection of Fulton County (HWCFC) comes in.
- Assist individuals to secure dental and optical care.
- Supply nutritious food to families facing hunger.
- Helps cover the cost of medications and medical supplies for those who otherwise cannot afford them.
Who is Eligible?
Fulton County residents ages 19 to 64 whose income is at or below 250 percent of current federal poverty guidelines. We are available to assist in calculating eligibility for potential clients.
Why do We Provide These Services?
The Health & Wellness Connection of Fulton County has a long history of serving our community. Previously, we operated as a clinic providing medical services to uninsured residents. As healthcare has evolved, our patient population’s needs have shifted. In response to that shift we have adapted our services to address the evolving needs of our community. Today we are dedicated to providing essential support to individuals and families navigating the complexities of healthcare access and food insecurity.
Impact
HWCFC has provided hundreds of patients/clients with medication and supplies, dental care, eye care, diabetic supplies, transportation fees and personal supplies over the past fifteen years.
