Our professional team of nurses and exercise specialists will develop an individualized program that will help you meet your specific goals, and get you on the path to living the strongest life you can. Our team is certified by the American Association of Cardiovascular and Pulmonary Rehabilitation.
Most programs require a physician’s order due to a variety of health concerns. Our staff is happy to obtain this for you. Any adult is eligible to participate in our maintenance/prevention programs, even if they have no significant medical diagnosis.

Cardiac Rehabilitation and Pritikin ICR
This program is physician referred and is designed for patients who have experienced a heart attack, angioplasty with stents, bypass surgery, valve replacement/repair, congestive heart failure (CHF), or heart transplant. This 12+ week program meets three times per week in the Graham Cardiopulmonary Rehab Department. Heart rhythm and blood pressure are monitored each session, and exercise progress is tracked by trained staff members. Nutritional assessments with a registered dietician, as well as education about coronary risk factor reduction, exercise, and stress management are included.

Pulmonary Rehabilitation
This program is physician referred and is designed for patients who have experienced the onset or acute flare-up of many lung diseases. Persons suffering from asthma, emphysema, chronic bronchitis and restrictive lung disorders are candidates for this program. Post-COVID complications may also qualify for this service. Patients meet three times per week and receive heart, blood pressure and oxygen level monitoring during each of the 36 sessions. Education for optimal self-care is emphasized. Oxygen is provided to patients when needed during exercise sessions.

Peripheral Arterial Disease (PAD): Supervised Exercise Therapy
Do you have claudication (cramping pain in the legs brought about by activity, due to blocked arteries in the lower extremities)? Exercise training has been shown to improve the walking ability in PAD patients that suffer from this type of pain. A physician referral is necessary for the “PAD SET” program. Each qualifying patient must have a face to face visit with their physician and receive information regarding cardiovascular disease and PAD risk factor reduction. The individualized program includes a closely monitored progressive exercise therapy. The patient may expect 36, 60- minute sessions over a 12 week period.

Supervised Maintenance/Prevention Exercise
Any person wanting exercise supervision or those who are just completing Phase II Cardiac or Pulmonary Rehab may maintain fitness in a familiar, monitored environment for as long as they need. Exercise classes are offered multiple times per week. This affordable self-pay program encourages long range adherence to exercise.
Cost: 3 visits per week = $45/month; 2 visits per week = $32/month
This Phase III Cardiopulmonary Rehab program is usually self-pay, however we now offer partnerships with Renew Active and Silver & Fit. We encourage participants with Medicare advantage plans to check their insurance for any maintenance exercise benefits.
