Health Information Management/Patient Records

One of the primary responsibilities of our health information management team is to fulfill requests for medical records from patients, insurers, physicians, and other healthcare providers while ensuring confidentiality and security of our patient’s medical record.

Accessing Medical Records

Your hospital medical records can be accessed by logging into the MyGraham Patient Portal, our confidential website that allows you to access information within your medical record. Patients may review instructions, test results, and information regarding your most recent hospital stay including your discharge medication list.

Patients who would like to request access to the MyGraham Patient Portal or who need assistance in locating information within the portal may contact a Graham Health System team member in one of the following ways:

• Call: 309-649-6800 Monday – Friday between 8:00 a.m. and 4:30 p.m.

Your physician office medical records can be accessed by logging into the Patient Portal, our Graham Medical Group confidential website that allows patients to access information regarding physician office visits. Patients may view scheduled appointments as well as laboratory and imaging results.

Patients who would like to request access to the Patient Portal or who need assistance in locating information within the portal may contact a Graham Health System team member by calling 309-647-0201, Extension 5100 Monday through Friday between 8:00 a.m. and 4:30 p.m.

Requesting Medical Records

In order to protect your privacy, we require written authorization before medical records are released. To obtain copies of medical records, patients will need to complete an Authorization for Release of Information form. The authorization must be completed, signed, and dated by the patient or the patient’s legal representative before copies will be provided.

Please allow up to ten business days to process your request. Records will be mailed to the address placed on the Authorization for Release of Information form. Records may also be picked up if prior arrangements are made with the Health Information Management department. To protect health information, a valid picture identification card (passport, driver’s license, state ID, etc.) must be presented when picking up medical records in person.

The Health Information Management Department is open between 8:00 a.m. and 4:30 p.m. Monday through Friday. Questions regarding medical records can be answered by calling:

Copies of birth or death certificates must be obtained through the Fulton County Clerk’s Office. They may be reached by calling (309) 547-3041 Ext 306 in Lewistown or at the Fulton County Health Department (309) 647-1134 in Canton.
